You and your child need to have held lifestyle extras, family extras or super extras cover for 12 months to be able to claim Orthodontic benefits.
How to claim
If you’ve paid the charge in full or are paying it off in instalments
You can submit your claim online to receive your benefit for the current financial year or in line with any instalment/payment dates. You will need to attach your receipt(s) in order to claim.
If you’ve paid upfront and want to claim over subsequent financial years
Please make sure you include alongside your receipt(s) written confirmation from your orthodontist or dentist to advise that the treatment is ongoing.
You can also complete a claim form and submit your form and receipts by logging in to your account.
Alternatively, post to: ahm health insurance, Locked Bag 4, Wetherill Park NSW 2164.
Don’t forget you need to retain an appropriate level of extras cover for the duration of the treatment.
Just so you know, we’ll only pay benefits towards dental and Orthodontic treatments that are delivered in person, and by a recognised provider. Dental and Orthodontic treatments are subject to annual limits, lifetime limits and waiting periods, check your product guide for more info.
