How to claim
If you’ve paid the charge in full or are paying it off in instalments
You need to attach your receipt(s) to a claim form to receive your benefit for the current financial year or in line with any instalment/payment dates.
If you’ve paid upfront and want to claim over subsequent financial years
Please make sure you include written confirmation from your orthodontist or dentist to advise that the treatment is ongoing.
Send your claim form and receipts to email@example.com including your member number in the subject field or post to: ahm health insurance, Locked Bag 4, Wetherill Park NSW 2164.
Please note: Orthodontics cannot be claimed online.
Don’t forget you need to retain an appropriate level of extras cover for the duration of the treatment.
