We will provide your tax info directly to the ATO, so there isn't a need to send out the statement for most of our members.
You can log in to your account or the ahm app to request your tax statement from 1 July. Alternatively, these can be requested by calling 134 246 (select option 1 then option 6) or contacting us via webchat.

If you require a copy of your statement before then, you can log in to your account or the ahm app to request your tax statement from 1 July.
Dependants included on a family policy are not subject to means testing for rebate purposes because they are not considered to have any share in the costs associated with the policy. However, a dependant child on your policy can use a copy of your tax statement if they need to complete the private health insurance policy details section of their tax return.
If you have a dependant on your policy, their name won’t appear on your tax statement, but the ATO will know they are on your policy based on the membership information we send to them.
Log in to your account or the ahm app after 1 July to request your tax statement.
Your private health insurance statement gives you information about your private health insurance premiums and private hospital cover. It will also help you complete the Medicare Levy Surcharge related items on your tax return.
The ATO will also calculate your correct entitlement to a private health insurance rebate when you lodge your tax return. Your entitlement to this rebate is income tested, which is based on your income for surcharge purposes.
We’ll send your details to the ATO after 1 July. Even though you’re no longer a member, the details can’t be generated until the financial year has ended. If you require a copy of your statement, you can order one by calling 134 246 and select option 1, then option 6.

Please note: If you had health insurance for some part of the financial year but didn’t make any payments between 1 July 2024 and 30 June 2025, this will be reflected in your statement. This may mean your statement does not show any payments.
Only payments receipted by ahm in the period 01/07/2023 - 30/06/2024 will be reflected, even if the payment was covering a period of membership within the financial year.
The premium amounts shown in column J reflect the total base premium before any Australian Government Rebate (AGR) is applied. Column K is the AGR amount applied to those payments. Where you claim the AGR as a premium reduction each time you pay Column J minus Column K will = the exact premiums you paid to ahm.
Where there are 2 adults on the policy, the total premium amount is halved so the amount shown in this case will be half the total paid.

The Australian Government Rebate (AGR) on private health insurance is a Government contribution to the cost of private health insurance. Australian residents who have private health insurance, qualify for Medicare and earn below a specified income threshold may be entitled to the AGR.

You can change or update your personal details until 30 June by logging in to your account. If you make changes after that date, your tax statement won't reflect your new details.
If you have updated your details and the changes aren't showing on your tax statement, give us a call. You can use the self-service option to generate a new tax statement that will show your updated details after 4 July.

The advantage of sending direct to the ATO is that members will be able to lodge their tax returns quicker because the info will be pre-filled in their returns.
